29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service.  (a)   Date of Violation: On or about 12-14-2016 and times prior thereto, located on the HRSD York River Treatment Plant Outfall and Diffuser Project, based out of 1901 Waterview Rd. Yorktown, VA 23692, while onboard Corman Marine Construction, Inc. barge, an employee was exposed to fire and electrical hazards from a MegaMC 2065A Fusion Machine electrical cabinet that had been damaged and temporarily repaired with a coat hangar resulting in the electrical cabinet enclosure door not being able to close to isolate the energized conductors, bus bars, terminals and other internal parts of the electrical equipment. The machines power source cable had been pulled from the cabinet damaging the strain-relief compression clamp.  The power cord contained no chaffing protection were it entered the electrical cabinet enclosure.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
