29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. On or about 12/15/2016 - 3402 Stanwood Dr., Huntsville, AL, the employer did not record the following workplace injuries on the OSHA 300 Log for the calendar year 2016. (a) On or about 11/7/2016, Logistics - An employee received a laceration on his left thumb from a knife resulting in another recordable case. (b) On or about 10/10/2016, Extrusion - An employee received a laceration on his left calf from a knife resulting in days away from work. (c) On or about 9/21/2016, Logistics - An employee received a back and knee sprain due to the forklift he was operating fell off a loading dock resulting in job restrictions. Abatement Certification and Documentation is required.
