29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:(a)    (a)  Southwest Roof Eave:  On December 16, 2016 and times prior, employees used a guardrail of a mobile scaffold to climb up and down 48 inches from the scaffold platform to the roof.  A ladder or stairway was not provided for employee access and exit from the roof.
