29 CFR 1926.451(b)(5)(ii):   The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:  a)  4 Venturi Court, Florham Park, NJ.  Employees were exposed to falls of up to 13 feet, 5 inches to the ground below from pump jack scaffolding as the one end of the platform extended over its supports by approximately 7 feet with no end guardrails blocking employees from accessing the cantilevered end.                               Violation observed on or about 11/18/2016.

29 CFR 1926.451(g)(1)(vii):   Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   a)  4 Venturi Court, Florham Park, NJ.  Employees were exposed to falls of up to 13 feet, 5 inches to the ground below from pump jack scaffolding as the scaffolding was lacking any forms of fall protection such as guard rails or netting and workers were not using any personal fall arrest systems.                                Violation observed on or about 11/18/2016.

29 CFR 1926.451(f)(3):   Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:   a)  4 Venturi Court, Florham Park, NJ.  Employees were exposed to falls of up to 13 feet, 5 inches to the ground below from pump jack scaffolding as  a competent person had not inspected the scaffold or its components for defects as shown by the end of the platform extending over its supports by several feet and as shown by the lack of fall protection for employees working on the scaffolding.                              Violation observed on or about 11/18/2016.

Employees were exposed to falls of up to 8 feet, 10 inches to the ground below from the sloped roof as no methods of fall protection were in use.
