29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about December 8, 2016 employees operating roller machines were exposed to amputation hazards and the employer did not ensure that ingoing nip points were effectively guarded: a. The top of the guard on tape machine #94 was approximately 42.5 inches from the ground and did not prevent employees from reaching over the guard and accessing the ingoing nip point. b. The top of the guard on Machine #3 was approximately 48 inches and did not prevent employees from reaching over the guard and accessing the ingoing nip point. c. The top of the guard on Machine #2 was approximately 48 inches and had gaps on the left and right large enough for employees arms to access the ingoing nip point. d. The top of the guard on Machine #1 was approximately 53.5 inches and had gaps on the left and right large enough for employees arms to access the ingoing nip point.
