29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. a) Fabrication area, Bay number 26: On or about 19 December 2016 and at times prior, the employer exposed employees to struck by hazards by not taking Forklift Number 07 out of service, in that the forklift had two frayed hydraulic hoses that control the mast lift.

Portable electric equipment and flexible cords used in highly conductive work locations (such as those inundated with water or other conductive liquids), or in job locations where employees are likely to contact water or conductive liquids, shall be approved for those locations. a) Test Area: On or about 19 December 2016, and at times prior, the employer exposed employees to electrical shock hazards of 120 Volts, in that an electrical extension cord was in contact with water while being used.
