29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:    (a)  Broes Electric, Inc. at 5541 Central Avenue, Boulder, CO:  On and preceding 11/28/16, the employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  Two employees of Broes Electric, Inc. performed work within an electrical switchgear box adjacent to energized parts.  During the work, a cable came into contact with an energized busbar.  An arc flash resulted from the contact.  The employer did not ensure that the apprentice electricians were provided with the training necessary to recognize and appropriately eliminate hazards associated with the energized busbars adjacent to which they were working.  This condition exposed the employees to an electrical hazard.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided and used:    (a)  Broes Electric, Inc. at 5541 Central Avenue, Boulder, CO:  On 11/28/16, the employer did not ensure that protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, and protective shields and barriers, were provided and used.  Two employees of Broes Electric, Inc. performed work within an electrical switchgear box adjacent to energized parts.  During the work, a cable came into contact with an energized busbar.  An arc flash resulted from the contact.  The apprentice electricians, working near energized electrical parts with a potential exposure to 480 volts, wore cotton clothing.  The employer did not ensure that the employees were provided with appropriate arc-rated clothing and protective equipment, and that the employees used this arc-rated clothing and protective equipment when exposed to electrical and arc-flash hazards.  This condition exposed the employees to electrical and arc-flash hazards.

29 CFR 1926.416(a)(1): The employer did not ensure that employees were not working in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employees were protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means:    (a) Broes Electric, Inc. at 5541 Central Avenue, Boulder, CO:  On 11/28/16, the employer did not ensure that employees were not working in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employees were protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means.  Two employees of Broes Electric, Inc. performed work within an electrical switchgear box adjacent to energized parts.  During the work, a cable came into contact with an energized busbar.  An arc flash resulted from the contact.  The apprentice electricians had been directed to place corrugated cardboard over the energized busbars.  The employer did not ensure that the energized busbars were effectively guarded through use of an appropriate insulating barrier.  This condition exposed the employees to an electrical hazard.

(b)   Broes Electric, Inc. at 9558 Weld County Road 7, Longmont, CO:  On and preceding 12/14/16,  the employer did not use an OSHA 300A form or equivalent form for recordable injuries and illnesses.
