29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:  a) 11 Wisteria Drive, Fords, NJ 08863, Building 17. The employer allowed his employee to operate a Ramset XT540 Powder actuated nail gun to secured 2 x 4 material to the facade of the exterior wall while not wearing any eye and or face protection exposing the employee to eye and face injuries from flying debris.     Occurred on 12/16/16

29 CFR 1926.451(f)(7): Scaffolds erected, moved, dismantled or altered were not performed under the supervision and direction of a Competent Person qualified in scaffolds:    a) 11 Wisteria Drive, Fords, NJ 08863, Building 17. The employer allowed his employee to work on and from Alum-A-Pole pump jack scaffold when the scaffold had not been erected under the supervision of a Competent Person exposing the employees to hazards associated with scaffold collapse.     Occurred on 12/16/16.

The employer allowed his employees to work on and from Alum-A-Pole pump jack scaffold installing 2 x 4 materials to the facade of the building when guardrails were not installed along all open sides and ends of walking/working platform exposing the employee to fall hazards in excess of 12 feet to the ground below. The employer allowed his employee to use the Alum-A-Pole work bench as a walking/working surface while installing 2 x 4 materials to the facade of the building exposing the employee to fall hazards in excess of 12 feet to the ground below. The employer allowed his employee to carry 2 x 4 materials up the ladder while not maintaining three points of contact when the employee could lose his balance and fall in excess of 12 feet to the ground below.
