29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Thermal Treatment Island; On or about November 23, 2016 and at times prior, employees were not using appropriate personal protective equipment when they were exposed to burn hazards. Zilkha Biomass Selma, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.132(a), which was contained in OSHA inspection number 1124499, citation number 1, item number 1 and was affirmed as a final order on August 17, 2016(date), with respect to a workplace located at 1256 County Road 78, Selma AL 36703.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) Thermal Treatment Island; On or about November 23, 2016 and at times prior thereto the employer exposed employees to potential hazardous energy sources in that the written procedure for the lockout/tagout of the Thermal Treatment Island system which includes the individual reactors did not have clear and specific steps for isolating, securing, and verifying that all hazardous energy sources (thermal) had been controlled. Zilkha Biomass Selma, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1124499, citation number 1, item number 2 and was affirmed as a final order on August 17, 2016(date), with respect to a workplace located at 1256 County Road 78, Selma AL 36703.
