29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:    At Warehouse 19, the small scale batch mixer displayed evidence of damage and was not removed from service prior to an employee using it before necessary repairs were made. The following items were observed to be damaged:  A.) The flexible cord had a missing grounding pin; and,  B.) The exterior insulation was pulled away from the electrical wiring entering the control box.
