29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  For the employees working in the trench on or about December 20, 2016 and times prior there to, at the intersection of Adams Street and Maryland Avenue on the south side of I-630 in Little Rock, Arkansas.  The employer did not locate spoil piles at least 2 feet from the edge of the excavation before allowing employees to enter the trench to manually dig to locate the natural gas line.  The spoil piles were located at the immediate edge of the trench above the location the employee was digging with a shovel in the excavation.  This exposes the employees to being struck by soil falling into the trench.
