29 CFR 1926.300(b)(1): Power operated tools that were designed to accommodate guards were not equipped with the guards when they were in use, to protect the operator and employees nearby from hazards such as ingoing nip point, rotating parts, flying chips, or sparks:     a)  Employees were exposed to laceration hazard when using a Makita electric grinder with an unguarded cutting disk during construction of concrete formwork. 29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from caveins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: (i) Excavations are made entirely in stable rock; or (ii) Excavations are less than 5 feet in depth and examination of the ground by a competent person provides no indication of a potential cavein:    a)  Employees were exposed to excavation collapse and falling rocks  when working in an unshored excavation in the basement. The 12 ft high vertical excavation had 5 feet of dirt and rocks at the top and bedrock lower down. 29 CFR 1926.703(a)(1):  For cast-in-place concrete: The employer did not ensure that formwork shall be designed, fabricated, erected, supported, braced and maintained so that it will be capable of supporting without failure all vertical and lateral loads that may be reasonably be anticipated to be applied to the formwork:    a) A concrete wall formwork in the basement collapsed when pumped full of concrete. Four employees were on a platform at the top of the 10 ft tall formwork and jumped to safety when they heard the wood bracing cracking. The employer did not ensure that the formwork was designed and braced sufficiently to support the weight of the concrete and forces related to concrete pumping and mechanical vibrating operations. 29 CFR 1926.703(a)(2):  For cast-in-place concrete:  The employer did not ensure that drawings or plans, including all revisions, for the jack layout, formwork (including shoring equipment), working decks, and scaffolds, shall be available at the jobsite:    a) A concrete wall formwork in the basement collapsed when pumped full of concrete. The employer built a single side of concrete form next to existing foundation wall to make the wall wider. The employer did not provide a plan to ensure loads were calculated and bracing was sufficient.    b) Additional concrete was poured to fill the form due to the uneven shape of the existing foundation wall; resulting in more weight the form had to support.  The employer did not provide a plan to ensure loads were calculated and bracing was sufficient.    c) The form was a single side form which does not allow for use of form system tie rods that maintain the shape of the form, resulting that more wood bracing was needed. The employer did not provide a plan to ensure loads were calculated and bracing was sufficient.
