29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about December 20, 2016, on the east side of the residential construction site where an employee was exposed to a struck-by hazard while using a Makita brand circular saw to cut pieces of wood without wearing eye protection.
