OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards:    (a) Lan Tec Area, Zone #5A: On 12/20/2016, the employer exposed employees to struck-by hazards from a pallet transfer car, in that employees using a roller conveyor as a walking/working surface to gain access to a machine operating aisle in order to view remaining pallets on the blow molding line.    Among other methods, one feasible and acceptable means of abatement would be to install physical or electronic barriers to prevent access onto the conveyors and into the moving machinery.

29 CFR 1910.303(b)(7)(iv): Electric equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  (a) Lan Tec Area, Zone #5A: On or about 12/21/2016, and at times prior, the employer failed to repair damaged junction box cover/strobe light attachment on the pallet transfer car, in that internal energized circuits were not covered.
