29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Jobsite, New Stairway: On or about December 19-21, 2016, employees were exposed to falling from about 11 feet to 25 feet while working on platforms in the new stairway.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jobsite: On or about December 19-21, 2016, the employer did not provide a fall protection training program to employees who were exposed to fall hazards. Employees were exposed to fall hazards ranging from about 11 feet to 25 feet while installing a new stairway.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge: (a) Jobsite, New Stairway: On or about December 19-21, 2016, employees would use the new stairway that they were installing and the new stairway had an open sideway and there was no stairrail system installed.
