29 CFR 1926.501(b)(14): The employer did not ensure that each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 m) above the walking/working surface, were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: Site a. On or about December 21, 2016, employees engaged demolition work on the exterior windows were not wearing fall protection and were exposed to falls of over 15 feet. Accolade Environmental Contracting, Corp. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926. 501(b)(10), which was contained in OSHA inspection number 417204, citation number 1, item number 1, and was affirmed as a final order on May 21, 2012, with respect to the workplace located at 45 S. Main St., Concord, NH.
