29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Site: 1677 43rd Street, Brooklyn, NY.    On or about 12/14/2016,    a) The 2nd floor work area had two unprotected leading edges.  Employees worked on the 2nd floor were exposed to a fall hazard of up to 18' to the level below.

It was used to access the 1st floor level from the ground floor level.

29 CFR 1926.451(e)(1): Portable ladder, hook-on ladder, attachable ladder, stair tower, stairway-type ladder, ramp, runway, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used to access scaffold platform more than 2 feet above or below a point of access.    IN THE ALTERNATIVE:    29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided.      Site: 1677 43rd Street, Brooklyn, NY.    On or about 12/14/2016,    a) Employee used a tubular welded frame scaffold as an access to the 2nd floor work area from the 1st floor.  The scaffold did not have safe means of access.  Employee climbed the cross bars and the frame of a walk-thru scaffold for access.
