29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) On or about November 1, 2016, two employees from the spraying area were re-assign to work in the lamination area to manually mix styrene-containing resin and Methyl Ethyl Ketone catalyst.  The employer failed to train these employees on the specific procedures they needed to follow to protect themselves from the exothermic reaction caused by mixing these two hazardous chemicals, exposing the employees to fire hazards.

29 CFR 1910.106(e)(2)(iv)(d): Flammable liquids were not drawn from or transferred into vessels, containers, or portable tanks within a building only through a closed piping system, from safety cans, by means of a device drawing through the top, or from a container or portable tanks by gravity through an approved self-closing valve:  a) On or about December 19, 2016, a 55 USGAL drum containing flammable resin was used to dispense into smaller containers with a quarter-turn valve instead of a self-closing valve exposing employees to flammable liquid vapors.

29 CFR 1910.106(e)(6)(ii): Grounding. Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), shall not be dispensed into containers unless the nozzle and container are electrically interconnected. Where the metallic floorplate on which the container stands while filling is electrically connected to the fill stem or where the fill stem is bonded to the container during filling operations by means of a bond wire, the provisions of this section shall be deemed to have been complied with:  a) On or about December 19, 2016, at the lamination area, an 55 USGAL drum containing flammable resin was used to dispense  the liquid into smaller containers and the employer did not ensure the drum was connected to ground, exposing the employees to an static electricity discharge that may result in a fire.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a) On or about December 19, 2016, at the small boxes lamination area, the access to a portable fire extinguisher was obstructed by materials and product, exposing to fire hazards such as smoke inhalation.
