29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  Trash Compactor Area:    The employer did not adequately guard the trash compactor at the point of operation, exposing employees to amputation hazards. One of the components of the interlock for the side-swing guard was broken off from its anchorage point so the interlock was no longer functional.
