29 CFR 1926.451(e)(1):  Portable ladder, hook-on ladder, attachable ladder, stair tower, stairway-type ladder, ramp, runway, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used to access scaffold platforms more than 2 feet above or below a point of access:       a)  Employees were exposed to a fall hazard of approximately 13 feet while accessing the work platform of a ladder jack scaffold.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:      a)  On December 21, 2016, employees working from a ladder jack scaffold were exposed to a fall hazard of approximately 13 feet.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  On December 21, 2016, employees performing roofing work were exposed to fall hazards of approximately 13.9 feet.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
