29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements are being followed. A. Norcold Inc. worksite located at 600 S. Kuther Rd., Sidney, Ohio: On or about December 16, 2016, an employee sustained an amputation injury while performing a mold change on the Modern Vac Forming machine. Employees are required to perform servicing and maintenance activities on the machine including but not limited to, mold changes, which require energy control procedures that have not been reviewed for deficiencies.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. A. Norcold Inc. worksite located at 600 S. Kuther Rd., Sidney, Ohio: On or about December 16, 2016, the employer did not ensure employees were trained to perform mold changes on the Modern Vac Forming machine utilizing adequate lock-out/tag-out procedures to minimize employees exposure to amputation hazards. Employees were not trained to isolate/de-energize all sources on the machine prior to performing mold changes.

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following actions and elements in paragraphs (d)(1) - (d)(6): A. Norcold Inc. worksite located at 600 S. Kuther Rd., Sidney, Ohio: On or about December 16, 2016, the employer did not ensure employees were protected while performing a mold change on the Modern Vac Forming machine. Employees were attempting to align the frame with the pneumatic energy not properly controlled, resulting in an amputation of an employee's middle finger when the air-actuated clamps closed.
