On November 17, 2016, on the balcony, employees performing framing and decking work were not protected from a 15 feet fall hazard by the use of a fall protection system.

On November 17, 2016, on the east side of the building, the ladder used to access and egress the balcony did not extend 3 feet above the landing, exposing employees to a 15 feet hazard.
