29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. On November 17, 2016, on the balcony, employees performing framing and decking work were not protected from a 15 feet fall hazard by the use of a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On November 17, 2016, on the east side of the building, the ladder used to access and egress the balcony did not extend 3 feet above the landing, exposing employees to a 15 feet hazard.
