On or about December 17, 2016 at 107 Jarson St. Oak Grove, Louisiana, the employer did not ensure that all personnel were properly notified that the power line ground connections would be removed then notified again before re-energizing power to the street, resulting in an employee suffering an electric shock with injuries.
