29 CFR 1910.269(m)(3)(xiii): The employer shall ensure that no one reenergizes the lines or equipment at a point of disconnection until all protective grounds had been removed, all crews who are working on the lines or equipment released their clearances, all employees are clear of the lines and equipment, and all protective tags are removed from that point of disconnection. On or about December 17, 2016 at 107 Jarson St. Oak Grove, Louisiana, the employer did not ensure that all personnel were properly notified that the power line ground connections would be removed then notified again before re-energizing power to the street, resulting in an employee suffering an electric shock with injuries.
