29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a) Red Roofing at 135 E 4th Ave Delta, CO: On 12/13/2016, the exposing employer did not ensure employees were wearing hard hats to be protected from the hazard of falling objects on a steep roof.  Employees were using forks to tear off roof shingles from a 18/12 steep pitched roof. Employees were working beneath each other on the roof, and on the ground where falling debris presented a hazard to the employees. This condition exposed employees to head injury from being struck by falling tools and materials.

This condition exposed employees to the hazards of projectiles to the eyes and from being struck by sharp objects.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:    (a) Red Roofing at 135 E 4th Ave Delta, CO: On 12/13/2016, the exposing employer did not ensure that a body belt of lanyard was attached to the boom or bucket when working from an aerial lift. Two employees were working on a Genie S-85 aerial lift while sheathing without connecting to the aerial lift. Condition exposed employees to a fall hazard of approximately 30 feet.

The employees were sheathing and tearing off shingles on a 18/12 steep pitched roof without the use of fall protection. This condition exposed employees to fall hazards of approximately 35 feet.

29 CFR 1926.503(a)(2): The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas: (i) The nature of fall hazards in the work area; (ii) The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used; (iii) The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; (iv) The role of each employee in the safety monitoring system when this system is used; (v) The limitations on the use of mechanical equipment during the performance of roofing work on low-sloped roofs; (vi) The correct procedures for the handling and storage of equipment and materials and the erection of overhead protection; (vii) The role of employees in fall protection plans; and, (viii) The standards contained in this subpart:  (a) Red Roofing at 135 E 4th Ave Delta, CO: On 12/13/2016, the exposing employer did not ensure employees were trained by a competent person regarding the fall hazards. Employees were sheathing and tearing off shingles and were not using fall protection. The employees had not been trained regarding the fall hazards of the site. This condition exposed employees to fall hazards of about 35 feet.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support:    (a) Red Roofing at 135 E 4th Ave Delta, CO: On 12/13/2016, the exposing employer did not ensure that the ladder extended at least three feet past the roof's eave. The employees used the ladder to ascend and descend from the ground to the roof. The ladder side rails did not extend three rungs up past the roof's eave and it was not secured. This condition exposed employees to fall hazards of 16 feet.
