29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers. Roof, 1505 Housatonic Street, Pittsfield, MA: On or about December 22, 2016 employees were observed entering and exiting the southern roof surface and were not protected from tripping into or stepping through holes (including skylights) by covers, guardrails or personal fall arrest systems. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
