29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure: a) Jobsite, 330 3rd Avenue, Pittsburgh, PA 15222: Observed on or about September 2, 2016, employees engaged in framing activities on the roof of the building were not protected from falling to level below. The employees were exposed to a fall hazard of approximately 7 feet 3 inches to the floor below. b) Jobsite, fourth floor window opening, 330 3rd Avenue, Pittsburgh, PA 15222: Observed on or about September 2, 2016, employees were observed moving materials through a fourth floor window opening. The window opening was not guarded. The employees were exposed to a fall hazard of approximately 30 feet to the concrete sidewalk below while working near the window opening.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Jobsite, 330 3rd Avenue, Pittsburgh, PA 15222: Observed on or about September 2, 2016, the employer did not provide a training program for each employee exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. Employees working at the jobsite were exposed to fall hazards of up to approximately 30 feet to the ground below.
