On or about December 27, 2016, in the basement, the employer failed to keep the aisles, stairways and work areas free of oil and water on the floor. The floor was not maintained in a clean and, so far as possible, dry condition thereby exposing employees to slip and fall hazards.

29 CFR 1910.26(c)(2)(vi)(d): Ladders exposed to oil and grease, equipment were not cleaned of oil, grease, or slippery materials. a. On or about December 27, 2016, in the basement, employees were required to use ladders covered with oil. The employer did not ensure that ladders exposed to oil were kept clean, thereby exposing the employees to slip and fall hazards.
