29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. a) Shop Floor at Press 25 and 26: On or about December 27, 2016 and at times prior, the employer exposed employees to slip & trip hazards in that while employees were operating the press machines rugs and mats were stacked on each other creating a trip hazard. b) Shop Floor at machine number AHR24: On or about December 27, 2016 and at times prior, the employer exposed employees to slip & trip hazards in that while employees were operating the press machines oil spills and leaks were not promptly cleaned up.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: a) North Side of building; Rollup Door: On or about December 27, 2016 and at times prior, the employer exposed employees to electrical and fire hazards in that an electrical disconnect for shop lights was not accessible to employees to turn off in the event of an emergency.
