29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s): 16 Louden Street, Worcester, Massachusetts; On or about December 28, 2016, employees were throwing roofing materials off the roof of a three story residential building to the ground below without the use of a chute.

29 CFR 1926.1053(b)(9): The area around the top and/or bottom of (a) ladder(s) was not kept clear: 16 Louden Street, Worcester, Massachusetts; On or about December 28, 2016 the base of the ladder used by employees to access the roof was not kept clear of debris that had been removed from the roof. This created slip and fall as well as puncture wound hazards from nails in the materials.
