29 CFR 1904.29(b)(3): How quickly must each injury or illness be recorded? You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. On or about December 29, 2016, the employer did not enter the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2016. (a) On December 14, 2016, an employee was hit in the head by the metal gate of a BMC (OTR) cage while sorting mail in the priority area. The employee declined medical attention, but the acting plant manager gave the employee three days off to "give her head time to recuperate." This lost time injury was not recorded on the OSHA 300 Log for 2016.
