Trash Pick-up Site - An employee was not wearing protective footwear while performing his duties resulting in the employee being hospitalized, due to a severely broken toe.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA. Facility - On or about December 20, 2016, an employee was hospitalized as the result of being struck by a garbage truck. The hospitalization was not reported to OSHA until 12:10 PM, on December 22, 2016.
