29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a.  On or about December 28, 2016, at the above addressed jobsite,  employees working on the second floor, below demolition activities, were not wearing hard hats to protect against falling materials and debris.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.    a.  On or about December 28, 2016, at the above addressed jobsite, employees had been provided fall protection training, however the employer failed to keep a written certification of record.

On or about December 28, 2016, at the above addressed jobsite, employees performing demolition  operations were exposed to falls of approximately eleven (11') feet eight (8"), created by the presence of a wall opening.
