29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:   a) Custom Etch / 1813 West State Street, New Castle, PA: On or about December 29, 2016 the employer did not have a written respiratory protection program. An employee would conduct blasting operations on parts while wearing a supplied air hood respirator. The employer was lacking a written respiratory protection program, including, but not limited to elements such as, selection of the respirator for the workplace; medical evaluations of employees using the respirator; cleaning, disinfecting, storing, inspection, repairing and maintaining the respirator; voluntary use of respirators; employee training on the use of the respirators, including use and limitations and evaluations of the program.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  a) Custom Etch / 1813 West State Street, New Castle, PA: On or about December 29, 2016, employees utilized a supplied air hood respirator for blasting operations.  The employees were not provided with a medical evaluation prior to the use of the hood respirator.
