29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. On or about December 15, 2016, an employee was exposed to caught-in and amputation hazards when performing servicing and maintenance on the Unimac. The employer failed to implement energy control applications steps as the machine was not shut down or turned off, in order to perform servicing work, per the 1910.147(d)(2) requirements. b. On or about December 15, 2016, employees were exposed to caught-in and amputation hazards when performing servicing and maintenance on the Unimac. The employer did not ensure interlocks were not being by-passed, during servicing work. ABATEMENT NOTE: The ANSI B11.19-2010: Performance Criteria for Safeguarding, Section 6. General Safeguarding Requirements, may be used for abatement purposes provided that the safety-related functions (interlocks) prevent employee exposure to hazardous energy.
