29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) In the Recovery Boiler Area, where the employer did not ensure that employees used goggles and face shields to protect the employees from caustic liquids while replacing the refractrometer. Employees were exposed to contact hazards.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: a) At the facility, where the employer did not train the employees on the appropriate work practices and procedures for refractrometer removals. Employees were exposed to contact hazards from the Black Liquor.
