Spanish Fort, Alabama: On or about January 3, 2017 and at times prior; the employer allowed his employees to use an extension ladder to gain access to the roof top and the rails of the ladder did not extend at least three feet above the landing area to provide safe access exposing his employees to a fall hazard of 10 feet 10 inches to the ground below.
