29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment were not used under the supervision of a qualified person: a) Conveyor dryer: On or about December 30, 2016 and at times prior thereto employees used anchorage attachment points as part of a personal fall arrest system that were located at the edge of the walking/working surface. This exposed the employees to a 12 foot fall to the concrete below. Note) Certification of corrective action is required for this item.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not be rigged such that an employee would neither free fall more than 6 feet (1.8 m), nor contact any lower level: a) Conveyor dryer jobsite: On or about December 30, 2016 and at times prior thereto an employee who was exposed to a 12 foot fall hazard used a personal fall arrest system which included a six foot shock absorbing lanyard anchored at the employees feet. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards to enable each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards: a) Jobsite: On or about December 30, 2016 and at times prior thereto employees used a personal fall arrest system for protection from a 12 foot fall hazard. The fall arrest system components and the installation would not have prevented the employee from contacting the lower level. The employer and employee were not able to recognize the difference between a fall arrest and fall restraint system and the hazards of using the wrong type of a fall protection system. Note) Certification of corrective action is required for this item.
