29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      a) Jobsite/Stairwell - The employer did not ensure that employees working around incomplete stair towers were protected from falling down into the unfinished basement, exposing employees to twelve (12) foot falls. An employee fell on 27 DEC 2016 as a result of the unprotected edge and sustained fatal injuries.     b)Jobsite/Elevator shaft - Unsecured wood planks or similar material was used on an unprotected edge leading to an elevator shaft. The planks were not secured and did not meet the requirements of a guardrail system exposing employees to fall hazards of approx. 12 feet. Observed on/or about 27 DEC 2016.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:      a) Jobsite/Basement access - A floor hole used to access a basement was not protected by a guardrail system or other means of fall protection exposing employees to fall hazards of approximately 12 feet.     b)Jobsite/Next to stairwell - A caution tape was used around a floor hole in lieu of guardrails or other means of fall protection exposing employees to fall hazards of approximately 12 feet.      Condition was observed on 27 DEC 2016.
