Employees did not take action to correct or minimize the hazard.  b) Conveyor dryer: On or about December 30, 2016 and at times prior thereto employees used a personal fall arrest system for protection from a 12 foot fall hazard.  The employer and employees did not recognize or acknowledge the difference between a fall arrest and fall restraint system and the hazards of using the wrong type of a fall protection system.   Note) Certification of corrective action is required for this item.
