29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about December 30 2016, employees were exposed to hand injuries and amputations while sanding the edges on metal parts and a suitable guard was not in place to protect the employees from the in going nip point.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On or about December 30, 2016, an employee suffered a work-related injury resulting in an amputation and the employer did not report the work-related amputation to OSHA within twenty-four (24) hours. The employer reported the incident on January 3, 2017.
