29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees:    (a) Location: 183 Foley Street, Somerville, MA    On or about 12/29/2016 there were unsupported utilities in the excavation exposing employees. There was an 8 inch gas line and 16 inch water main that were left unsupported in the excavation.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      (a) Location: 183 Foley Street, Somerville MA    On or about 12/29/2016, employees were using an 8 foot step ladder as a means of egress from the excavation and the step ladder did not extend three feet out of the trench.

29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe:    (a) Location 183 Foley Street Somerville, MA    On or about 12/29/2016, employees were exposed to hazards struck by due to a trench box that was being supported by a chain and hook with a sprung latch.    The J. Marchese & Son was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.251(a)(1), which was contained in OSHA inspection number 991978, citation number 01, item number 1A and was affirmed as a final order on 09/10/2014, with respect to a workplace located at 6 Cross Street, Beverly, MA 01915.
