29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about December 21, 2016 employees were exposed to caught in hazards including crushing or amputation where the employer failed to ensure that the guards on a power press break prevented the hands or fingers of the operator from being in the danger zone during the machine cycle.
