29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.    On or about 04 January 2017, the employer failed to examine the powered industrial trucks as least daily for any condition that could adversely affect the safety of the vehicle. Employees were exposed to a crushing hazards and struck by hazards.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).    The Compass Group USA, Inc. DBA: Canteen Vending was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(1)(ii), which was contained in OSHA inspection number 996291, citation number 01, item number 3a and was affirmed as a final order on 03/25/2015, with respect to a workplace located at 955 Sivert Dr. Wood Dale, IL. 60191.    On or about 04 January 2017, the employer failed to ensure that each operator had successfully completed training to operate the companies Powered Industrial Vehicles. This exposed employees to a struck by hazard.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.178(l)(2)(ii): Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    The Compass Group USA, Inc. DBA: Canteen Vending was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(2)(ii), which was contained in OSHA inspection number 947359, citation number 2, item number 1 and was affirmed as a final order on 01/14/2014, with respect to a workplace located at 2890 Commerce Park Drive, Madison WI.53719.    On or about 04 January 2017, the employer failed to ensure that each operator had successfully completed the combination of formal instruction, to include class room instruction, practical training, and an evaluation of the operators performance. This exposed employees to a struck by hazard.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.  On or about 04 January 2017, the employer failed to properly mark the aisles and passageways for the employees to move around the facility. This exposed employees to struck by hazards from passing PIV's (Powered Industrial Vehicles).  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.178(a)(6): The user shall see that all nameplates and markings are in place and are maintained in a legible condition.  On or about 04 January 2017, the employer failed to ensure that all Powered Industrial Vehicles used in material handling operations, had the required nameplates and or markings maintained in a legible condition. Nameplate was missing on the Toyota Powered Industrial Vehicle. This exposed employees to a struck by hazard.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
