29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:    Sheet metal department, Roll Form and Cut-Off Press - on or about December 21, 2016, the employer did not conduct adequate periodic inspections to identify and correct inadequacies/deviations of lockout procedures regarding the methods operators used to perform setup (servicing) tasks. Operators fed material through the in-going rollers using a single-hand jog controller that did not protect their fingers and hands from unexpected startup and associated caught-in hazards.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    Sheet metal department, Roll Form and Cut-Off Press - on or about December 21, 2016, roll former operators performing setup (servicing) tasks while feeding material through in-going rollers (point-of-operation and associated danger zone) with a single-hand portable jog controller were not required to apply energy controls in the following sequence prior to starting work:     (a) prepare for shut down by recognizing energy hazards and its controls,  (b) turn off and shut down the machines,  (c) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (d) affix lockout or tagout devices to each energy isolating device,  (e) render safe all potentially hazardous stored or residual energy,  (f) verify that isolation and de-energization of the machine or equipment have been accomplished.    Roll former operators had their fingers and hands exposed to caught-in hazards likely to result in serious injury from unexpected start-up while feeding the coil strip into the in-going nip points created between the rollers.  "Manual mode" selection to prevent unexpected run mode/start-up did not provide an effective alternative protective measure.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    Roll Form and Cutoff Press, operator's desk: on or about January 4, 2017, employees were exposed to fire hazards where relocatable power taps and a three-way adapter cord were improperly used. One relocatable power tap was permanently secured to a desk and not connected to permanently installed branch-circuit receptacle outlet. This equipment is not listed for such use.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection:  Sheet metal department: on or about January 4, 2017, the employer's "Lockout Audit of Proficiency" form developed to satisfy certification requirement of this standard did not identify the machine or equipment on which the energy control procedure was being utilized.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.255(b)(5): The hazard of flying sparks was not, wherever practical, eliminated by installing a shield guard of safety glass or suitable fire-resistant plastic at the point of operation:  Welding area, Ideal mesh welder - on or about January 4, 2017, operators and helpers were handling material in a danger zone during welding operation that exposed them to hazards of flying sparks because safeguards were not provided.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
