29 CFR 1910.106(e)(3)(v)(a): The enclosed processing building ventilation was not arranged with discharge or exhaust to a safe location out of the building.  This shall be accomplished by natural or mechanical ventilation with discharge or exhaust to a safe location outside of the building. Provision shall be made for introduction of makeup air in such a manner as not to short circuit the ventilation. Ventilation shall be arranged to include all floor areas or pits where flammable vapors may collect.   Rubber Polymer Company, Cumming, GA- In the mixing room, employees were exposed to flash fire and explosion hazards when the employer failed to adequately ventilate the mixing equipment where flammable vapors could accumulate within the enclosed room. The employer used flammable chemicals such as, but is not limited to lactol, resins, and heptane.  The ventilation used only one fan to ventilate the three mixing vessels, on or about 11/01/16.

29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors:   Rubber Polymer Company, Cumming, GA- A flash fire occurred in the mixing room on 10/25/16.   Employees were exposed to flash fire and explosion hazards when the employer failed to prevent a flammable atmosphere from developing within equipment used within the mixing room during normal process operations while dispensing flammable liquids into a mixing tank. The employer did not contain ignition sources and vapors , on or about 10/25/16.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
