OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by hazards:    A)    Autoneum North America, Inc., located at 645 Lallendorf Road, Oregon, OH 43616: On or about November 7, 2016, employees were exposed to stuck-by hazards from projected machine components while working near the South Edge Grinder, located at the Feilong Line.  Serious injury could result to employees if the equipment malfunctioned and exploded, causing pieces of the grinders to strike employees in the vicinity.    B)     Autoneum North America, Inc., located at 645 Lallendorf Road, Oregon, OH 43616: On or about December 23, 2016, employees were exposed to stuck-by hazards from projected machine components while working near the North Edge Grinder, located at the Feilong Line.  Serious injury could result to employees if the equipment malfunctioned and exploded, causing pieces of the grinders to strike employees in the vicinity.    Among other methods, some feasible and acceptable methods to correct this hazard would be to:  ·         use alternative equipment, such as chop fans, to process material   ·         install a different model grinder, which is designed for the type of material being processed, and designed to contain internal components in the event of a catastrophic failure.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    A)    Autoneum North America, Inc., located at 645 Lallendorf Road, Oregon, OH 43616: On or about December 23, 2016, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from nip-point hazards created by moving parts such as, but not limited to, the oven conveyor and the transfer table at the Feilong Line. Employees were exposed to ingoing nip-points and caught-in hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    A)    Autoneum North America, Inc., located at 645 Lallendorf Road, Oregon, OH 43616: On or about December 23, 2016, the employer did not properly train employees to lockout/tagout equipment such as, but not limited to, the North Edge Grinder located at the Feilong Line. Employees did not control all hazardous energy sources such as, but not limited to, electrical before employees conducted activities such as, but not limited to, unjamming the grinder. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.      B)     Autoneum North America, Inc., located at 645 Lallendorf Road, Oregon, OH 43616: On or about December 23, 2016, the employer did not properly train employees to lockout/tagout equipment such as, but not limited to, the oven conveyor located at the Feilong Line. Employees did not control all hazardous energy sources such as, but not limited to, electrical before employees conducted activities such as, but not limited to, unjamming the conveyor. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.      Autoneum North America, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1910.147(c)(7)(i)), which was contained in OSHA Inspection # 1112707, Citation number 1, Item number 2, and was affirmed as a final order on March 16, 2016, with respect to a workplace located at 1150 North Wynn Road, Oregon, OH 43616.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     A)    Autoneum North America, Inc., located at 645 Lallendorf Road, Oregon, OH 43616: On or about December 23, 2016, the employer did not ensure that employees implement energy control procedures on the equipment such as, but not limited to, the North Edge Grinder located at the Feilong Line, prior to conducting activities such as, but not limited to, unjamming the grinder. The North Edge Grinder was not isolated from the electrical energy sources prior to engaging in this maintenance activity. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.      B)     Autoneum North America, Inc., located at 645 Lallendorf Road, Oregon, OH 43616: On or about December 23, 2016, the employer did not ensure that employees implement energy control procedures on the equipment such as, but not limited to, the oven conveyor located at the Feilong Line, prior to conducting activities such as, but not limited to, unjamming the conveyor. The oven conveyor was not isolated from the electrical energy source prior to engaging in this maintenance activity. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.      As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from amputation hazards while employees conduct servicing activities such as, but not limited to, unjamming the grinders.      Autoneum North America, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1910.147(d)), which was contained in OSHA Inspection # 1112707, Citation number 2, Item number 1, and was affirmed as a final order on March 16, 2016, with respect to a workplace located at 1150 North Wynn Road, Oregon, OH 43616.      Autoneum North America, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard (1910.147(d)(3)), which was contained in OSHA Inspection # 1063461, Citation number 1, Item number 2, and was affirmed as a final order on August 18, 2015, with respect to a workplace located at 645 Lallendorf Road, Oregon, OH 43616.
