29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On the west side of the construction site, employees not wearing eye and face protection while using a chop saw to cut steel material were exposed to struck-by hazards and eye injuries.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On the west side of the construction site, employees elevated by a Genie lift to install metal framing material were not provided fall arrest systems exposing them to unprotected fall hazards of approximately 9 to 12 feet.
