29 CFR 1910.266(g)(3): Each vehicle operator and maintenance employee did not comply with the operating and maintenance instructions: a) Logging Truck: On or about December 28, 2016, the employer did not ensure passengers used the provided seat and seatbelt which exposed employees to injuries associated with falling off, being struck by, and being run over by the logging truck.

29 CFR 1910.266(i)(7)(i): The employer did not assure that each employee, including supervisors, had received first-aid and CPR training meeting at least the requirements specified in Appendix B: a) Logging site located near Marsh Cove, Demopolis, AL: On or about December 29, 2016 and at times prior thereto, the employer did not ensure all employees had completed the required First-Aid and CPR training before being allowed to conduct logging operations which negatively impacted the coverage of emergency care for injured employees at the logging site.
