29 CFR 1926.502(h)(1): The employer did not designate a competent person to monitor the safety of other employees and/or did not ensure that the safety monitor complied with the requirements listed in (h)(1)(i) through (h)(1)(v) of this section: On or about 01/05/2017 at the Shelden Oak II Re-Roofing Project, the safety monitor did not know that all employees must be in visual sight of the safety monitor at all times while the safety monitor system was in use as the fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about 01/05/2017 at the Shelden Oak II Re-Roofing Project, employees were using an extension ladder to access the roof deck and the ladder did not extend 3 feet past landing.
