29 CFR 1904.39(a)(2): The in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident was not reported within 24 hours. (a) Site: 27 Eagle Terrace, Holden, MA 01520, garage: On 11/9/2016 an employee using a circular saw sustained a finger amputation and as of 1/5/2017 the employer had not reported the incident to OSHA.
